  • Work has been authorized.
  • Staff is adequately rested.
  • Staff is using proper PPE.
  • Staff has task experience.

Hazards

  1. Panic during abandonment
  2. Incorrect lowering of lifeboat(s) can cause serious personnel injuries / fatalities
  3. Incorrect use of life saving appliances
  4. Incompetent personnel to perform
  5. Inadequate communication
  6. Safety routines not followed
  7. Life boat / equipment failure
  8. Emergency escapes obstructed / blocked
  9. LSA Equipment failure

Control Measures

  1. Follow instructions, Master’s orders to be strictly followed, execute as in an emergency, abandon ship check list.
  2. Team leader, clear instructions, ensure all crew know their role when abandon ship order is given, abandon ship checklist.
  3. Life saving manual, safety inspections & inventory, proper maintenance and training, initial and on the job training.
  4. Training and drills procedures, crew evaluation reports, crew certification authentication checks, interview process prior employment.
  5. Establish communication with emergency and support squads as instructed by the Master, abandon ship checklist.
  6. Familiarization and training, instructions / checklist for abandon ship.
  7. Planned maintenance schedule / SOLAS maintenance manual.
