- Work has been authorized.
- Staff is adequately rested.
- Staff is using proper PPE.
- Staff has task experience.

- Panic during abandonment
- Incorrect lowering of lifeboat(s) can cause serious personnel injuries / fatalities

- Incompetent personnel to perform

Control Measures
- Follow instructions, Master’s orders to be strictly followed, execute as in an emergency, abandon ship check list.
- Team leader, clear instructions, ensure all crew know their role when abandon ship order is given, abandon ship checklist.
- Life saving manual, safety inspections & inventory, proper maintenance and training, initial and on the job training.
- Training and drills procedures, crew evaluation reports, crew certification authentication checks, interview process prior employment.
- Establish communication with emergency and support squads as instructed by the Master, abandon ship checklist.
- Familiarization and training, instructions / checklist for abandon ship.
- Planned maintenance schedule / SOLAS maintenance manual.
- Safety manual, preventive maintenance plan, instructions / inspections.
